In this integrated role, you will act as both Case Manager and Support Coordinator, emphasizing the dual responsibility of detailed case management combined with strategic support coordination. This role is critical in bridging the gap between strategic planning and operational execution, ensuring that our participants receive the highest quality of care and support.

KEY RESPONSIBILITIES
- Evaluate and manage the ongoing needs of participants through comprehensive risk management and personalized service planning.
- Facilitate and participate in care team meetings with allied health professionals, focusing on a person-centered approach to care.
- Drive across Victoria to meet with potential participants, assessing their needs, and managing their seamless integration into our services.
- Maintain accurate and comprehensive case notes and records, adhering to NDIS standards and organizational policies.
- Communicate effectively with various stakeholders, building strong relationships.
- Identify appropriate supports, engage with new and existing support providers, and work towards achieving identified outcomes and goals.
- Holistically address points of crisis, barriers, and participant risks, designing solutions to manage these risks.
- Stay abreast of and adapt to new processes and changes in NDIS policies, demonstrating a commitment to continuous professional development.
SKILLS
- Significant understanding of information technology to manage and report on participant care effectively.
- A strong solution focus and creative thinking ability.
- Strong foundation in both case management and support coordination within the NDIS framework, with experience in coordinating community-based supports.
- Required travel throughout Victoria with a valid driver’s license.
- Preferred background in health or social sciences, particularly with experience in handling complex care needs.

- NDIS worker screening check (required)
- First Aid and CPR License (required)
Job Types: Full-time, Permanent
Salary: $70,000.00 – $85,000.00 per year
